New Patient Information
Thank you for letting us help you with your dental needs.
ON
YOUR FIRST VISIT, a thorough dental examination will be performed.
This will consist of a complete series of x-ray, digital photographs, a visual
examination and the use of any diagnostic aids that may be indicated. Once
the necessary information is gathered, the doctor will discuss with you your
dental needs, the appropriate treatment, and an estimate of costs involved.
FINANCIAL ARRANGEMENTS are handled by our financial
administrator. She will explain in detail the various payment methods available
to help you in your planning. Dr. Cook is a participant in two insurance plans,
United Concordia and Guardian. Payment is due in full at the time
of service. We accept Visa, Mastercard, Discover and American Express.
A DENTAL APPOINTMENT is a reservation of your
time, the doctor’s time and the time of the entire office staff. We
will do our very best to be ready for you at your scheduled appointment time.
Should it be necessary for you to reschedule an appointment, kindly give us
at least two working day's notice (48 hours) so that your reserved appointment
may be given to someone else in need of dental services. We appreciate this
courtesy. Failure to give 48 hours notice may result in a broken appointment
fee of $25. Multiple cancellations will prevent you from receiving another
appointment.
STATE OF THE ART EQUIPMENT AND TECHNOLOGY Dr. Cook utilizes the latest
computer technology including digital patient records, digital x-rays, and
electronic claim filing.
